Code of Conduct
Below is our code of conduct that needs to be signed by both parent and athlete before they are able to participate with West Cheer. Please let your coaches know if there are any questions.
Physicals, Tryout Registration & APP Test Registration
Athletes must have an updated physical by the time they tryout. If you are unsure if your physical is still in good standing, please contact a coach or the athletic department. Physicals are due to the athletic office by 8/9/19. (Physical Update Forms cannot be dated any earlier than 7/29/19.)
The following options are available if your son or daughter wants to participate on an athletic team:
1. Obtain a physical at the school.
2. Your child may get a sport physical by their private physician. The physical MUST be completed on the Health Appraisal physical form.
If you choose to have the physical done by your private physician or HealthWorks, the completed physical form MUST be delivered prior to the start of practice to the school nurse (during the school year) or Athletic office (during the summer).

Hours: M-F 8:30 am - 4:00 pm (summer hours Mon-Thurs 7:30 am to 3:00 pm)

All athletes must complete a health update form, IF THEY DID NOT HAVE TO RENEW THEIR PHYSICAL PRIOR TO TRYOUTS..
All necessary forms can be found below.
The online registration process MUST BE COMPLETED BY a PARENT/GUARDIAN- THROUGH PARENT PORTAL. REGISTRATION OPENS 7/24/19. Please note: Families with multiple students must complete an online registration form for each of their students.
1. Please complete the registration for athletics.
2. Please ensure the parent/guardian and emergency contact information is accurate.
NO athlete will be able to tryout if they are not registered by AUGUST 9TH
APP TEST-7th and 8th graders ONLY
All 7th/8th graders must pass the APP test before the are eligible to try out. Athletes must complete the following:
Step 1: Request/Permission form submitted to the Director of Athletics) SUMMER SEASON DUE 7/26
Step 2: Medical Clearance - A valid physical with Tanner level must be submitted to the district. This must be completed BEFORE the physical fitness portion of the process. (attachment below)
Step 3: Physical Fitness Testing (set up by appointment only).
